Handicare has several policies in place that help assure that the children enrolled are cared for appropriately and in a healthy environment. These policies affect both the children and the employees of Handicare. Parents and the staff at Handicare create a partnership in the implementation of these policies, so that they can accomplish the goal of maintaining a healthy environment.
The intake packet that a parent initially receives contains all of the health forms that we require and are kept in the child�s confidential file.
Parents are responsible for reporting to Handicare any changes to these health forms when they occur. We request that these forms be updated annually by the parents.
Parents are responsible for providing in writing the following information at enrollment and on an annual basis:
- Names, phone numbers and addresses of their child�s physician and dentist.
- Which local hospital their child should be taken to in the event of an emergency.
- Consent to obtain emergency medical care, along with current insurance information.
- A current physical record for children under 5 (health assessment for school aged children). These must be received before the child can start our program. A health statement from a physician is an acceptable substitute for an annual physical, once the initial physical is submitted. We will provide the health statement that you should have your physician sign and then return to us.
- A current immunization record. Also, updating the child�s immunizations in our records, as the child receives them.
